I understand that the provider decides whether treatment is appropriate and may decline to prescribe, ask for more information, or recommend in-person care. I understand that any medication prescribed may have side effects, that results vary and are not promised, and that I should read the Important safety information and the pharmacy label. I agree to follow my provider's instructions, to report side effects or changes in my health, and to tell my provider before stopping a medication. I can withdraw this consent at any time by contacting support@saphirevitality.com, except for care already provided.
